Our multi-disciplinary health care team focuses on all aspects of care relating to both pre- and type 2 diabetes; the goals of management being to either prevent or delay the progression from pre-diabetes to overt type 2 diabetes, and to prevent or delay the onset or progression of any associated complications. This is all while maintaining quality of life.
As part of our service delivery, individual consultations with members of our team are available for all persons with pre- or type 2 diabetes who are referred to our service, as required.
However, all persons with type 2 diabetes who are referred to the Georgian Bay Family Health Team will also be given the opportunity to participate in a group program. This is run over the course of two mornings, covering aspects such as: what is type 2 diabetes?, blood glucose monitoring, dietary intake, physical activity, medications, psychological well-being and foot care.
With our mission being to provide unparalleled collaborative health care to improve the overall health of the Georgian Bay population, we also undertake outreach work within the community. This is in the form of presentations surrounding diabetes prevention and management, for example.
