To submit your name for an appointment, you can go to the GUIDANCE OFFICE, fill out a slip a for locked box. You will be called out of class sometime during a clinic day.
NO PARENTAL INVOLVEMENT REQUIRED AND CONFIDENTIALITY IS GUARANTEED!!!
